Le ms. provient de l'abbaye cistercienne Notre-Dame de Châalis (Oise), dont l'ex-libris contemporain figure au f. 3v, suivi d'un anathème: "Liber Sancte Marie Karoliloci". Ce ms. correspond à l'article n°26 du catalogue des mss. de Châalis dressé au tournant des XIIe-XIIIe siècles (éd. H. Martin, Catalogue des manuscrits de la Bibliothèque de l'Arsenal, t. 8, Paris, 1899, p. 440-445). Il a ensuite appartenu au prieuré de Saint-Martin-des-Champs (Paris) qui récupéra de nombreux débris de la bibliothèque de Châalis au début du XVIIe siècle.
