Aux ff. 1-2, anciennes gardes renfermant un fragment d'acte du bailliage d'Evreux, daté du 20e jour de janvier 1536.
Congrégation des Feuillants de la rue Saint-Honoré (Paris). Le ms. a été donné aux Feuillants par Barrois en 1766 d'après une note figurant au f. 2v: "Dono D. Barrois Bibliopolae Parisiensis hic codex ad Bibliothecam fuliensium S. Bernardi transiit, 1766." Il provient du monastère chartreux de La Rose (Seine-Maritime): "Domus Rosae ordinis Carthusiansis secus Rothomagum" (XVIe s., f. 2v). A la suite de cet ex-libris, un possesseur a rajouté la mention suivante: "Sum D. Bulteau pastoris S. Laurentii cuius bibliothecam datis libellis 8000 l. m... (?)" (XVIIe s.). Les anciennes gardes relatives au bailliage d'Evreux (cf. supra) confirment la provenance normande du manuscrit.
